A PSA test measures the amount of prostate-specific antigen in the blood. Healthy men have low amounts of PSA in the blood, but the amount of PSA normally increases as the prostate enlarges with age.

PSA levels increase with prostate cancer, but can also increase with inflammation of the prostate gland. There are also other factors that can temporarily raise PSA levels. Finding and treating prostate cancer early is important because it significantly improves the prognosis.

Today, the Swedish National Board of Health and Welfare does not recommend general PSA screening because elevated PSA levels can have causes other than cancer. This can create unnecessary worry, excessive investigation and overtreatment. Many men with elevated PSA levels may need follow-up even though they do not have prostate cancer, and in some cases may be diagnosed with cancer and receive treatment with side effects unnecessarily.

New, more precise tests as alternatives to PSA are under development and could in the future reduce the number of false alarms and unnecessary biopsies, but today PSA is what is available.

Personally, I believe PSA tests are of great value for detecting prostate cancer early and should be encouraged, especially in men between 50 and 75 years of age and in anyone with a family history of prostate cancer. At the same time, it is important to remember and explain that an elevated PSA level can have causes other than cancer that must be ruled out.
